My two cents: clutch wear happens during slippage, not when it's fully engaged or disengaged (but there may be exceptions). And especially when feeding it a lot of power during slippage.
So the three scenarios I can think of would be
- frequent use of power while half clutch (of the kind where you think 'but should it not be able to handle that?')
- setup problem causing incomplete engagement. (why I'm wary of lowering clutch pedal)
- hardware fault. Maybe hydraulic.
